PAT Screening At Skyline R-2
The Skyline R-2 School District will have P.A.T., Early Childhood and kindergarten screenings for children from birth through age five April 30 – May 4. Preschool students already enrolled in Skyline will be screened during school hours when there is an opening.
Screenings are a free service offered to anyone in the district regardless of whether you choose to enroll in any of the programs. This screening is required for children who will enroll in Early Childhood or Kindergarten for the 2012-13 school year. To make an appointment for a 3, 4 or 5 year old, please call Skyline School at 683-4874.
Screenings will take approximately one hour. Forms will be available to enroll your child during your appointment. This enrollment form must be signed by the parent / guardian of the child. (Results of screening need to be shared with the parent or guardian, so we prefer they bring their child to screenings.)
Your child’s birth certificate, Social Security card, and immunization records are required for Early Childhood and kindergarten enrollment.
Developmental screenings for children ages 6 months to 3 years will be conducted also. These screenings are provided by Parents as Teachers and are free; enrollment in the P.A.T. program is not required. If you have questions about P.A.T. call Kim Crabtree at 746-4101 ext. 129.
